Job description
Signify Health, part of CVS Health®, is seeking a per diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. The visits are about 45 minutes, with a focus on medication/history review and physical evaluation. No prescribing or lab orders required.
The role requires travel within licensed states, virtual visits as needed, and a base hourly rate plus incentives. Part-time, 12 hours/week, with benefits for eligible hours.
