Job description
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations for members in their homes. Visits last about 45 minutes and include history review, physical exam, and selective tests as indicated by the health plan.
You will travel locally, join a national network of clinicians, and use an iPad with a streamlined workflow. Prior licensure, board certification, and multi-state credentials are valued; a part-time schedule with 12 hours weekly
