Job description
Signify Health, part of CVS Health, is seeking a Per diem Clinician (NP or PA) to deliver in-home health evaluations, traveling to members’ residences and bridging gaps in care. You’ll assess health needs, review medications and history, and use an iPad-based workflow without altering treatment plans unless directed by the health plan.
Expected weekly hours are 12, with a part-time schedule and a pay range of $37.66–$81.11 per hour.
