Job description
Signify Health is seeking a Per diem Clinician (NP or PA) to conduct in-home health evaluations across multiple states. The role includes travel to member homes, using an iPad-based clinical workflow, and providing thorough health assessments without prescribing or altering treatment plans.
The position is part-time with a base hourly rate plus incentives per visit. Licensure must be active in coverage areas, with multi-state licenses preferred and malpractice coverage provided.
