Job description
Signify Health, part of CVS Health®, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for members in their own homes. The visit includes history, meds review, a physical exam, and potential additional tests as ordered by the health plan.
The position is part-time with 12 hours weekly, offering a base hourly rate plus incentives, malpractice coverage, and supplies.
