Job description
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (NP or PA) to perform in-home health evaluations, engaging with people in the comfort of their own homes and helping bridge gaps in care.
The visit lasts about 45 minutes, with a focus on history, medication review, and physical evaluation. You’ll use an iPad workflow and are not required to prescribe medicines or order labs.
