Job description
8205 Signify Health Medical Associates, PLLC is seeking a Full-Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations. The role involves visiting members in their homes, reviewing medications and medical history, and conducting physical assessments using a streamlined clinical workflow.
Clinicians will travel locally within licensed states, may conduct virtual visits, and provide comprehensive diagnostic and preventive services as needed.
