Job description
Matrix Medical Network invites experienced Nurse Practitioners and Physician Assistants to join our in-home health assessment team. You will conduct comprehensive CHA visits at patients' homes or SNFs, reviewing medical history, medications, and risks to identify care gaps and improve outcomes.
The role requires NP/PA licensure, ability to travel 80–90% across the state and neighboring areas, and offers a base salary plus incentive plan, plus a $7,500 sign-on bonus and a comprehensive benefits
