Job description
Monogram Health is seeking Advanced Practice Providers (NP/PA) to deliver high-quality in-home care to patients with multiple chronic conditions. You will work within an integrated team, providing continuity of care in patients' homes and via telehealth, with a focus on value-based, patient-centered care.
The role includes travel to patient homes, performance of comprehensive assessments, disease education, and coordination with PCPs and specialists.
