Job description
Monogram Health seeks Advanced Practice Providers (NP/PA) to deliver in-home care to patients with multiple chronic conditions, including diabetes, hypertension, CKD, heart failure, COPD, and more. You will join an integrated care team, perform comprehensive assessments, educate patients, and coordinate with PCPs and specialists.
This field-based role requires travel to patient homes and telehealth support, with a focus on a value-based, patient-centered care model.
