Job description
Monogram Health is seeking Advanced Practice Providers (NPs or PAs) to deliver in-home care for patients with multiple chronic conditions, including diabetes and heart failure. You will join an integrated care team, performing initial exams, care coordination, and telehealth visits as needed.
Responsibilities include comprehensive assessments, patient education, and collaboration with PCPs and specialists to improve quality of life while reducing hospitalizations.
