Job description
OptumCare is seeking a Nurse Practitioner or Physician Assistant to perform annual in-home health assessments for Medicare Advantage and other plan members in Mitchell, Yancey and Avery Counties, NC. This field-based role emphasizes preventive care, education, and collaboration with primary care providers to improve outcomes.
You will conduct comprehensive home visits, help close gaps in care, educate patients on disease management and medications, and coordinate with an interdisciplinary team.
