Job description
Black Women's Mental Health Institute seeks a clinician to conduct thorough in-home health assessments for adult and geriatric members, up to one hour per visit. You will review medical history, current health, medications, and social factors to close care gaps and improve outcomes.
Responsibilities include performing assessments, ordering tests, collaborating with physicians and care teams, and delivering education during home or skilled nursing facility visits.
