Job description
Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform comprehensive in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs to improve outcomes.
