Job description
Monogram Health is seeking an Advanced Practice Provider (NP/PA) to deliver in-home care to patients with multiple chronic conditions in Cleveland, OH. The role requires travel to several states for approximately two weeks per month, with mileage and hotel accommodations covered per policy.
You will join an integrated care team, perform comprehensive assessments, and provide continuity of care in patients' homes and via telehealth.
