Job description
Signify Health is hiring Full-Time Clinicians (NPs or PAs) to deliver in-home health evaluations across a defined coverage area. Visits are 45 minutes with history, exam, and optional diagnostic testing, using an iPad workflow. Travel up to 100% may be required with possible overnight stays and multi-state licensure.
You’ll bridge gaps in care for health plan members. Active, unrestricted licenses, board certification, and the ability to obtain multi-state licensure are required.
