Job description
CVS Health, via Signify Health, seeks a Part Time Clinician (NP or PA) to perform in-home health evaluations for members in their homes. You’ll engage with patients during visits designed to review medical history, perform a physical, and provide personalized health insights.
Visits typically run ~45 minutes with optional virtual sessions. The role requires active, unrestricted licensure, board certification, and multi-state licenses preferred.
