Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to provide In-Home Health Evaluations in patients' homes. You will review medications and medical history, perform a physical evaluation, and coordinate care with the health plan without prescribing or ordering labs unless directed.
Travel is required locally and within licensed states; the role offers a 40-hour work week with comprehensive benefits, including health, dental, and vision insurance, PTO, and a 401K
