Job description
Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for members in their homes. You will review medical histories, conduct physical exams, close care gaps, and document findings in our technology platform. Collaborating with interdisciplinary teams, you’ll help develop care plans, identify social determinants of health, and support value-based care goals.
