Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive in-home evaluations for vulnerable patients. You’ll review histories, reconcile medications, assess social needs, and document findings in EHRs to close care gaps and support value-based care.
