Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive in-home health assessments for adult and senior populations. You will review histories, medications, and social determinants, conduct focused exams, and develop evidence-based care recommendations that support value-based care.
