Job description
Signify Health is hiring an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for members in their homes. You will assess medical, functional, and social needs; update medication lists; identify care gaps; and document findings in an electronic platform to support value-based care. Partner with care teams to inform care plans, educate patients and caregivers, and connect them with community resources.
