Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive evaluations for adults and seniors in their homes. You will review medical histories, reconcile medications, identify care gaps, and develop evidence-based care plans using Signify’s technology platform. Partnering with interdisciplinary teams, you’ll support value-based care, educate patients and caregivers, and coordinate follow-up services.
