Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for vulnerable patients in their homes. You’ll review histories, medications, and social factors, develop evidence-based care recommendations, and collaborate with primary care and care management teams. Using modern technology and data, you’ll support value-based care, document in EHRs, and educate patients and caregivers.
