Job description
Matrix Medical Network seeks Nurse Practitioners to perform in-home Comprehensive Health Assessments (CHA) across the country. Visits last up to one hour, reviewing health history, medications, adherence, and risks to identify care gaps and improve outcomes.
You will conduct geriatric assessments, order testing, and collaborate with PCPs, family members, and other clinicians, with services at members' homes or SNFs.
