Job description
Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare members in a non-prescribing, field-based role. You will collaborate with primary care providers to close care gaps and educate patients.
The position offers a $10,000 sign-on bonus for external candidates, flexible scheduling (8am–8pm across a 7-day week), and no on-call, weekends, or holidays. Compensation ranges from $109,500 to $164,000 plus benefits.
