Job description
Optum HouseCalls is seeking a Nurse Practitioner to perform in-home health assessments for Medicare Advantage members in the Wilkes-Barre area. This field-based, non-prescribing role focuses on preventive care, member education, and identifying gaps in care during home visits. You will work a flexible schedule between 8 a.m.
and 8 p.m., with no on-call and no holidays. This position emphasizes one-on-one patient interactions and collaboration with members' primary care providers to improve care
