Job description
Signify Health, part of CVS Health®, seeks a Part-time Nurse Practitioner to perform in-home health evaluations, visiting members in their homes and bridging gaps in care. Visits typically last 45 minutes and involve history, physicals, and optional tests as ordered by health plans.
You will use an iPad with a clinical workflow, travel locally as needed, and may conduct virtual visits. Requires active, unrestricted licensure and board certification; multi-state licenses preferred.
