Job description
CVS Health Corporation through Signify Health seeks a part-time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations in members’ homes across licensed states. You will assess medical history, perform physical evaluations, and coordinate with health plans to ensure appropriate care pathways.
The role requires active licensure, board certification, and willingness to travel; virtual visits may be needed.
