Job description
Signify Health is seeking a Full Time Clinician (Nurse Practitioner or Physician Assistant) for in-home health evaluations across Louisiana. This role focuses on building trusted patient relationships and delivering personalized health evaluations directly in patients' homes.
The candidate is expected to have an active license, board certification, and preferably multi-state licenses. Offer includes a comprehensive benefits package and the anticipated weekly hours are 40.
