Job description
Signify Health, part of CVS Health, is seeking a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations. The clinician will visit patients in their homes, bridging gaps in care and delivering personalized health assessments.
The role includes a ~45‑minute visit, medication and medical history review, a physical evaluation, and potential additional diagnostic tests ordered by the health plan. Travel locally and occasional virtual visits are expected.
