Job description
Matrix Medical Network seeks a Nurse Practitioner to conduct comprehensive in-home health assessments. You will review medical histories, medications, and risks, documenting findings per Matrix guidelines, with visits at members' homes or SNFs.
You’ll collaborate with physicians, case managers, and other care team members to close care gaps and improve outcomes. A valid NP license and prescriptive authority are required, with flexible scheduling and field-based work across the region.
