Job description
Matrix Medical Network seeks experienced Nurse Practitioners to perform Comprehensive Health Assessments in patients' homes or facilities. Each visit lasts up to one hour and includes review of medical history, medications, and risk factors to identify care gaps.
You will collaborate with physicians and care teams to improve outcomes. We offer flexible scheduling, competitive pay based on completed assessments, mileage reimbursement, 401(k), and ongoing education.
