Job description
Matrix Medical Network seeks skilled Nurse Practitioners to perform comprehensive in-home health assessments for members, documenting medical histories, medications, and risks. Visits can last up to an hour, with care tailored to each member at their residence or in SNFs.
Providers collaborate with PCPs and care teams, administer tests as needed, and travel across the assigned state and neighboring states. Flexible scheduling and supportive onboarding are offered.
