Job description
Matrix Medical Network delivers in-home clinical assessments nationwide, empowering clinicians to improve health outcomes for older adults and at-risk individuals. As an NP, you will conduct comprehensive in-home assessments, review medical history and medications, administer testing, and coordinate with PCPs and care teams.
You will provide services at members’ residences, including homes and Skilled Nursing Facilities, with flexible scheduling across Sundays through Saturdays.
