Job description
Matrix Medical Network is hiring Nurse Practitioners to perform comprehensive in-home health assessments across clients in their homes or nursing facilities. Each visit lasts up to one hour and focuses on health history, current medications, adherence, and social factors to close care gaps.
Providers work with a care team including physicians and case managers, delivering education and follow-up while traveling to members’ homes as needed. Competitive pay and scheduling flexibility are offered.
