Job description
Signify Health, part of CVS Health, is seeking a Full Time Clinician (NP or PA) to perform in-home health evaluations. You will connect with patients in their homes, review medical history, perform physicals, and deliver diagnostic services under health plan guidelines.
This role supports national coverage with travel to licensed states, and virtual visits when needed. A clinical license, board certification, and multi-state eligibility are required or preferred.
