Job description
Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for members across the state.
During visits of up to one hour, you review medical history, medications, and living conditions, coordinating care with PCPs and documenting findings per Matrix guidelines.
You will collaborate with physicians, case managers, social workers, and family members, with flexible scheduling and potential FT opportunities.
