Job description
Matrix Medical Network seeks an experienced Nurse Practitioner to perform comprehensive in-home health assessments, reviewing medical history, medications and social factors to identify care gaps. You will collaborate with physicians, case managers and caregivers to deliver optimal patient education and follow-up.
Responsibilities include ordering tests, documenting outcomes, and providing care in members’ homes or SNFs.
