Job description
Matrix Medical Network seeks a Nurse Practitioner to perform Comprehensive Health Assessments in patients' homes, reviewing medical history, medications, and social factors. Each visit lasts up to one hour and aims to identify care gaps and coordinate follow-up with PCPs.
You will administer point-of-care and lab tests, collaborate with physicians, case managers and families, and travel up to 80–90% across the state and possibly neighboring states. A valid NP license is required.
