Job description
Matrix Medical Network seeks Nurse Practitioners to perform comprehensive health assessments in home settings, traveling to Livingston, TX and surrounding areas. Visits may last up to an hour with detailed health review and documentation.
Your role includes coordinating with PCPs, case managers, and other care team members, conducting geriatric assessments, and presenting findings for care gaps closure at each visit.
