Job description
Matrix Medical Network in Wisconsin is seeking a Nurse Practitioner to join our in-home assessment team. You will review medical history, medication adherence, and social factors during visits that typically last up to one hour, often at the member’s residence.
You’ll work with PCPs, case managers, and family members to close care gaps, perform point-of-care tests, and document findings. We offer flexible scheduling, competitive pay, mileage reimbursement, and continued education opportunities.
