Job description
HarmonyCares is seeking a Nurse Practitioner (or PA) to deliver home-based primary care to high-acuity patients. You will work in a team-based, physician-led model aimed at quality, not volume, with a focus on addressing each patient’s unique needs.
The role requires state licensure as NP/PA, DEA eligibility, CPR/BLS, and board certification. A driver’s license and clean driving record are necessary for home visits.
