Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive in-home evaluations for vulnerable patient populations. In this field-based role, you’ll review medical histories, assess chronic conditions, address social determinants of health, and develop care plans that close gaps and support value-based care. You’ll document visits in our modern technology platform, collaborate with interdisciplinary teams, and use data-driven insights to improve outcomes and reduce costs, while contributing to a mission-focused, flexible, and inclusive culture.
