Job description
Signify Health is seeking an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for patients in their homes. In this field-based role, you’ll conduct detailed histories, physical exams, and medication reviews, leveraging Signify’s technology and data to identify care gaps and support value-based care. You’ll collaborate with interdisciplinary teams, educate patients and caregivers, and document findings in EHR systems.
