Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for patients in their homes. You’ll review histories, medications, and social factors; identify care gaps; and develop data-driven care plans that support value-based care. Using Signify’s technology platform, you’ll document visits, collaborate with interdisciplinary teams, and coordinate follow-up services.
