Job description
Signify Health seeks an In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations in patients’ homes. You will review medical histories, conduct physical exams, address chronic conditions, and identify care gaps, using our advanced technology platform to capture accurate, data-driven insights.
