Job description
Signify Health seeks a Nurse Practitioner to conduct in-home health evaluations for diverse, often vulnerable patients. In this field-based role, you’ll perform comprehensive assessments, manage chronic conditions, review medications, and identify care gaps to support value-based care programs. You’ll document visits in an EHR, collaborate with care coordination teams, and use data-driven insights to guide interventions.
