Job description
Signify Health is seeking a full-time In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for Medicare and vulnerable populations in their homes. You will review histories, assess chronic conditions, reconcile medications, and identify gaps in care while documenting findings in an EHR. Working with interdisciplinary teams, you’ll support value-based care, patient education, and care coordination.
