Job description
Signify Health is seeking a per diem In-Home Nurse Practitioner or Physician Assistant to perform comprehensive health evaluations for adult and senior patients in their homes. You will complete detailed histories, physical exams, and medication reviews, document findings in our EMR, and identify care gaps to support value-based care. Working autonomously in the field, you’ll use mobile technology and data-driven protocols while partnering with interdisciplinary teams.
